        i)    Putting it in the repeat prescription receptacle in the surgery reception, or;

        ii)   Posting it to the surgery.

       If you would like us to post the completed prescription back to you please include a stamped addressed envelope

We will need to have the following information :

Patient's name, address, date of birth, phone number

For each item we will need to know: Name of drug, its strength, how many time a day it is taken, its form (e.g. tablets, capsules, liquid, etc), the amount needed (e.g. 56 tablets), any reason why you have asked for a variation (e.g. Dr Smith, the consultant at the hospital has increased my dosage)
